This is a message from the California Department of Education (CDE), Early Education and Support Division (EESD).


Attention: Executive Officers and Program Directors of Child Development Programs

The CDE/EESD Field Services Office wishes to inform you that the Confidential Application for Child Development Services form CD-9600 has been updated and posted on the CDE Web site for your use starting immediately.

The updates affect the following:

Section 11(A): Family Eligibility: cash aid recipient is now a separate category.

Section V: Certification and Signature of Parent/Caretaker: additional items:

  • The addition, Item 8, is family asset not exceeding $1,000,000 per Child Care and Development Block Grant (CCDBG) Act, Section 658P (4)(B);
  • The new Item 1 is the verification of the Single Parent Self-Certification-Documentation of the presence or absence of a parent where a parent must initial under penalty of perjury;
  • The addition of a signature box for parent B. Please note: two signatures are not required. This is also clarified on page 2 of the instruction pages.

Although CDE/EESD CD-9600 forms are not mandated, any form used by contractors to apply for service must include the current CCDBG certification. Contractors who use forms other than the CD-9600 are therefore required to update their forms to reflect the changes in the current CD-9600.

Contractors are expected to use or include the current required information in all new certifications or re-certifications starting immediately.
